Job description
Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments for vulnerable and Medicare Advantage populations in patients’ homes. You will review medical histories, conduct physical exams, identify care gaps, and document findings in an EMR to support value-based care. Collaborating with interdisciplinary teams, you’ll use data-driven insights to inform care plans, close quality measures, and coordinate follow-up.
