Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for vulnerable populations in their homes. In this field-based role, you will assess medical histories, medications, chronic conditions, and social determinants, then document findings using Signify’s technology platform. You’ll develop evidence-based care plans, educate patients and caregivers, and collaborate with interdisciplinary teams to support value-based care.
