Job description
Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations, visiting members in their homes to bridge gaps in care. You will assess health needs, discuss plans, and document findings in the clinical workflow using an iPad.
Travel to licensed states is required, and no prescribing is necessary. Candidates must hold an active, unrestricted license, board certification, and multi-state licenses preferred.
