Job description
Signify Health, part of CVS Health, is seeking a Part-time Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The visit lasts about 45 minutes and includes medication/history review, a physical exam, and optional tests ordered by the health plan.
You will travel locally within licensed states, and may conduct virtual visits as needed, joining a national clinician network dedicated to improving health outcomes.
