Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You will assess medical history, conduct physical exams, reconcile medications, and identify care gaps for often vulnerable populations. Using Signify’s technology and analytics, you’ll document findings in EHRs, develop evidence-based recommendations, and collaborate with interdisciplinary teams to advance value-based care.
