Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to provide comprehensive health evaluations for vulnerable patients in their homes. You will perform detailed assessments, review histories and medications, and document findings in EHRs while leveraging data and analytics to close care gaps and support value-based care.
