Job description
Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to conduct comprehensive health evaluations for vulnerable patients in their homes. In this field-based role, you’ll perform detailed histories, physical exams, and medication reviews while leveraging Signify’s technology and analytics to identify risk, close care gaps, and support value-based care. You’ll collaborate with interdisciplinary teams, educate patients and caregivers, and document thoroughly in digital tools.
