Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health assessments for vulnerable patients in their homes. You will evaluate medical history, medications, and social factors; document findings in EHR systems; and collaborate with interdisciplinary teams to close care gaps and support value-based care. Using data-driven insights and modern technology, you’ll educate patients, coordinate care, and help reduce avoidable hospitalizations while contributing to a mission-focused, innovative, and inclusive culture.
