Job description
UnitedHealth Group is seeking Nurse Practitioners and Physician Assistants for the Optum Home & Community Care team in Oklahoma. The role includes in-home assessments, care coordination, and advanced illness conversations, with no nights, weekends or holidays.
You will travel daily to patient homes within your area, provide preventive care, document thoroughly, and collaborate with an interdisciplinary team to improve access to care.
