Job description
Advantmed is seeking a dedicated Nurse Practitioner to join our full-time provider network for in-home wellness and risk adjustment assessments across the state. You will ensure accurate documentation and support high-quality, value-based clinical care.
Travel to members’ homes will be required to perform comprehensive evaluations, wellness visits, and chronic-condition assessments, including history, exams, and preventive screenings.
