Job description
Meritus Health is building a high-autonomy house-calls program for patients needing wound care and longitudinal primary care in homes and at our Home Health clinic. You will own your patient panel, practice at the top of license, and serve as the clinical backbone of the model.
You will provide in-home visits for wound care and primary care, with potential clinic sessions at the Home Health department. Manage a defined patient panel, coordinate with Home Health nurses and physicians, and enjoy
