Job description
CRS & Home Health Advantage is seeking a licensed Physical Therapist to visit patients in their homes on a part-time, full-time, or per diem basis. You will implement home exercise programs, conduct safety evaluations, and promote falls prevention while improving ROM and muscle education.
The role involves working with one patient at a time, coordinating with an interdisciplinary team to meet clinical outcomes and empower families in their care plans.
