Job description
BayCare HomeCare in Lakeland, FL is seeking a Home Health Physical Therapist to provide hands-on care in patients’ homes. The role requires evaluating patients, developing therapy plans, and coordinating with a multi-disciplinary team to deliver high-quality home health services.
