Job description
Pathways Healthcare in Concord, MA is seeking a licensed Physical Therapist for our home health and hospice services. The role focuses on delivering PT per physician plans, evaluating functional status, and supporting patients at home with dignity and purpose.
You will collaborate with physicians, nurses, and families, supervise PT assistants, manage discharge planning, and contribute to a compassionate, patient-centered care program. A MA license and at least 2 years of experience are preferred.
