Job description
CenterWell Home Health in Prescott, AZ is seeking a licensed Physical Therapist to deliver in-home care, helping patients regain mobility and independence through personalized therapy.
You will plan and administer prescribed PT, assess strength, develop rehab plans, guide PTAs and aides, and document progress while communicating with physicians and families. Requires active PT license and CPR certification.
