Job description
Pathways Healthcare in Concord, MA is seeking a licensed Physical Therapist to join our home health and rehabilitation team. You will deliver PT services per physician plan of care, assess function, and support patients at home with dignity and safety.
You will collaborate with a care team, train families, supervise PT aides, and contribute to discharge planning. A minimum of two years' PT experience, MA license, and a valid driver's license are required.
