Job description
Impact Healthcare LLC is seeking a Home Health Physical Therapist to join our Contra Costa County team. The PT will deliver patient-centered care in patients' homes, assess function, and design individualized treatment plans in collaboration with physicians and nurses.
The ideal candidate holds a current state license, at least 1 year in home health, strong communication, and bilingual English/Spanish skills.
