Job description
Bergen Care Home Health is seeking a PRN Physical Therapist to provide in-home physical therapy in Paramus, NJ. You will evaluate mobility, design individualized treatment plans, and deliver hands-on care to support independence and safety.
You will educate patients and families, document progress, and coordinate with the care team to ensure smooth transitions between services. Prior home health or rehab experience is preferred.
