Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to provide in-home care, helping patients regain strength and independence through personalized, functional treatment plans.
You will work with the Clinical Manager and care team to assess needs, implement rehab programs, document progress, and report outcomes to physicians and families. Expect travel to patient homes with a 40-hour work week.
