Job description
Altenheim Home Health Care in Lorain, OH is seeking a Physical Therapist to provide in-home rehabilitative care as part of an interdisciplinary team. You will assess function and implement therapy per Plan of Care.
The role requires a valid Ohio PT license, 2+ years of experience, and a valid driver’s license with access to a vehicle. Strong communication with patients and families is essential.
