Job description
CenterWell Home Health is seeking a licensed Physical Therapist to help patients regain strength, mobility, and independence in their homes, reporting to the Clinical Manager. You will deliver personalized care focused on functional improvement to enhance quality of life.
Responsibilities include planning and administering PT, evaluating progress, guiding treatment goals with physicians, supervising assistants, documenting thoroughly, and communicating with families and care teams.
