Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home rehabilitation, helping patients regain strength, mobility and independence, while collaborating with a clinical team to develop individualized care plans.
You will assess patients, document progress, supervise PTAs and home health aides, and report outcomes to physicians and family. A valid state PT license and CPR certification are required.
