Job description
CenterWell Home Health, a Humana company, is seeking a Home Health Physical Therapist to deliver in-home therapy, improve function, and empower patients to regain independence. You will plan treatments, assess progress, and coordinate with physicians, therapists, and families to meet individualized goals.
Qualified candidates hold an unrestricted Physical Therapy license and CPR certification, with prior teamwork in healthcare; one year of PT experience is preferred.
