Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to plan and administer prescribed physical therapy in patients' homes, helping regain strength, mobility and independence through personalized care. You will assess and document patient progress, supervise PTAs and Home Health Aides, and coordinate with physicians and the care team to meet treatment goals.
