Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to provide in-home therapy, helping patients regain strength and independence. You will plan, implement, and document PT treatments in patients' homes, collaborating with a multidisciplinary team to meet rehabilitative goals.
