Job description
CenterWell Home Health seeks a passionate Home Health Physical Therapist to deliver in-home physical therapy, assess patient needs and document progress. You will plan personalized rehabilitation aimed at maximizing functional outcomes while coordinating with physicians and care teams.
Responsibilities include guiding PTAs and HHAs, documenting progress, and communicating with families. A valid PT license and CPR certification are required, with a focus on collaborative, patient-centered care.
