Job description
CenterWell Home Health, part of Humana Inc, is seeking a Home Health Physical Therapist. You will plan and administer prescribed physical therapy in patients' homes to restore function and independence, working under the Clinical Manager.
Responsibilities include assessing patient strengths, designing rehab programs, guiding PTAs and aides, documenting progress, and communicating with physicians, care teams and families. Requires active PT license and CPR; one year experience preferred.
