Job description
Health Marketing is seeking a Home Health Physical Therapist to assess patients and provide rehabilitation services in home settings across California. You will develop individualized care plans, deliver skilled interventions per physician orders, document progress, and educate patients and families while collaborating with multidisciplinary teams.
