Job description
CenterWell Home Health, part of Humana Inc, is seeking a Home Health Physical Therapist Assistant. You will implement the PT plan at patients' homes, performing therapeutic exercises and modalities under supervision.
You will document care, coordinate in-services, and ensure infection control and safety standards are met. The role requires a PT license, valid driver's license, CPR certification, and strong teamwork with geriatric care knowledge.
