Job description
CVS Health, Signify Health in Wisconsin, is seeking a Per Diem Physician to perform in-home health evaluations. The role focuses on connecting patients with the right care in the comfort of their homes, while maintaining safety and quality standards.
You will conduct visits around 45 minutes, review medical history, assess needs, and document findings in our clinical workflow. Travel locally within licensed states is required, with virtual visits as needed.
