Job description
Lucet is hiring providers to deliver comprehensive in-home and community-based care, working within Monroe County, NY. You will assess patients, manage chronic conditions, and document in EMR systems while collaborating with interdisciplinary teams to ensure coordinated care.
Position requires an active state license, related certifications (FNP/AGNP/PA), DEA and BLS, and a clean driving record. Travel within 50 miles of home address is expected.
