Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive health evaluations for vulnerable patients in their homes. You will perform detailed assessments, review medical histories and medications, and identify gaps in care, leveraging Signify’s technology platforms and analytics. Partnering with primary care providers and interdisciplinary teams, you’ll create evidence-based care plans, support chronic disease management, and coordinate follow-up services.
