Nurse Practitioner - In Home Primary Care - Rhode Island.

Job Details
Weekly Estimated Pay: $3,685 - $3,950.
Shift: 8-hour days, 5 shifts per week.
Duration: 13 weeks.
Start Date: ASAP.
Job Requirements
Graduation from an accredited health professions school with active national board certification as a Nurse Practitioner.
Active licensure in the state of Rhode Island.
Active DEA licensure and state-specific controlled substance registration, as applicable.
Certification in Basic Life Support (BLS).
Ability to travel as needed with proof of liability and property damage insurance on vehicle used.
Minimum of 1 year of experience in a relevant healthcare field.
Responsibilities
Deliver clinical care both in-person at patients homes.
Provide compassionate, individualized, and holistic patient care, including comprehensive primary care, urgent care, and chronic condition management.
Collaborate with a multidisciplinary care team for transitions of care support and complex care management.
Engage in professional development opportunities and collaborate with experts in geriatric medicine and palliative care.
Participate in after-hours on-call coverage as needed.
