Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults and seniors in their homes. You will review histories, medications, and social factors; perform focused exams; and document findings in our technology platforms to close care gaps and support value-based care. Collaborating with interdisciplinary teams, you’ll educate patients and coordinate follow-up while using data to improve outcomes.
