Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive evaluations for patients in their homes. You will perform detailed health assessments, review histories and medications, evaluate functional and social needs, and develop evidence-based care plans that support value-based care. Using EMR and data-driven insights, you’ll identify care gaps, document thoroughly, and collaborate with interdisciplinary teams to improve outcomes for vulnerable populations.
