Job description
Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive clinical assessments in members’ homes across the Dallas area. You will evaluate medical history, conduct physical exams, close care gaps, and document findings in our technology platform to support value-based care.
