Job description
Signify Health is hiring a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults and seniors in their homes. You will conduct head-to-to-toe exams, review histories, reconcile medications, and document findings in an EHR to support value-based care and risk adjustment. Collaborating with interdisciplinary teams, you’ll identify care gaps, educate patients, and make evidence-based recommendations.
