Job description
Signify Health is seeking an In-Home Physician Per Diem to perform comprehensive clinical evaluations for patients in their homes. You will conduct histories and physicals, review medications, assess social determinants, and identify care gaps to support value-based care programs. Using Signify’s technology platform, you’ll document findings, coordinate with health plans and primary providers, and recommend follow-up care.
