Job description
Signify Health seeks a part-time In-Home Physician to perform comprehensive clinical evaluations for adults in their homes. You will assess medical, functional, and social needs; review medications; interpret diagnostics; and document findings in an EMR to support value-based care programs. Collaborate virtually with interdisciplinary teams to close care gaps, refine care plans, and coordinate services for vulnerable populations.
