Job description
Professional Case Management is seeking RN-Case Managers to provide in-home nursing visits for former Nuclear Weapons Workers with chronic and terminal illnesses in Colorado. You will evaluate care, supervise caregivers, and coordinate services with regional staff.
The role supports flexible scheduling and meaningful, one-on-one patient relationships. Visits are typically 1–2 hours weekly, including vitals, safety checks, and medication planning.
