Job description
Home & Health Care Management in Northern California is seeking a Registered Nurse to join an interdisciplinary care management team overseeing in-home and community-based services. This full-time role is based in our Redding office and requires travel to client homes.
You will review medical records and plans of treatment, coordinate with providers, social services and case managers, and monitor quality indicators to improve outcomes.
