Job description
Alignment Health, LLC is seeking an Advanced Practice Provider (NP/PA) to manage complex, high-risk seniors in a home-based care model. You will collaborate with physicians, case managers, and social workers to deliver coordinated care and ensure HIPAA/compliance standards.
The role involves 3.5 days of in-home assessments and 1.5 days remote work, with emphasis on cognitive/safety screenings and end-of-life discussions when appropriate. EPIC experience preferred, driving integrated patient care.
