Job description
Evolve Therapy Services is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Queens Village, NY. This full-time/part-time/PRN role focuses on delivering high-quality rehabilitative care to residents and collaborating with an interdisciplinary team.
You will assess residents, develop and implement treatment plans, document in CMS-compliant NetHealth EMR, and participate in ongoing professional development to maintain licensure.
