Job description
Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Waverly, NE. This is a full-time (also flexible hours, part-time or PRN) opportunity to provide comprehensive PT services within an interdisciplinary team.
Responsibilities include evaluation, planning, implementing therapy programs, documentation in CMS-compliant NetHealth EMR, and ensuring resident care quality. Requires a BS in Physical Therapy and active PT licensure in the state.
