Job description
Evolve Therapy Services in Massena, NY is seeking a Physical Therapist for an in-house Skilled Nursing Facility with flexible Full Time, Part Time, or PRN opportunities.
You will provide PT services, develop treatment plans, and document per CMS standards while collaborating with an interdisciplinary team to optimize resident outcomes.
Active Physical Therapy license in state(s) of practice and a BS in Physical Therapy are required; ongoing continuing education to maintain licensure is encouraged.
