Job description
Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Lawrence Township, NJ. This PRN position supports resident care with comprehensive PT services and interdisciplinary collaboration.
You will evaluate residents, develop treatment plans, document per CMS expectations in NetHealth EMR, and communicate findings to residents and families. Current NJ PT licensure and ongoing CEU are required to maintain certification.
