Job description
Partners Health Management is seeking an In-Reach Specialist to engage and educate adults with serious mental illness about community-based services and supported housing options in North Carolina. You will help inform individuals about tenancy supports, rental assistance, and Medicaid-related services while promoting recovery-oriented activities.
Responsibilities include assessing interest in housing, conducting on-site visits, and coordinating with peer supports and guardians.
