Primary Care Physician

The primary care physician will provide health assessments, diagnoses of acute and chronic illnesses, as well as formulate treatment plans. Qualifications: Doctor of Medicine Degree or its foreign equivalent. Physician License from the State of Indiana or at the time of hire.
