Job description

Bloom Healthcare is seeking a Primary Care NP/PA to deliver high-quality home- and community-based primary care in the Denver area. You’ll manage complex conditions with support from an interdisciplinary team and advanced data systems.

Ideal candidates hold a Master’s degree, active Colorado licensure, and a passion for compassionate, autonomous, patient-centered care. Expect a structured schedule, meaningful autonomy, and strong clinical collaboration.
