Job description
CenterWell Home Health, a Humana company, seeks an LPN to join its patient-focused team. You will provide skilled nursing care at patients' homes, under an RN, and help implement care plans that promote recovery and independence.
Reporting to the Clinical Manager, you will educate families, maintain records, coordinate with physicians, and participate in discharge planning. The role requires a current nursing license, a valid driver's license with insurance, CPR, and 2 years of LPN/LVN
